Subject: Appeal for Refund - [App Name] - Order #[Number]

Body: To the App Store Review Team, I am writing to request a refund for [App Name] purchased for $[Amount]. The app fails to perform its core advertised function. Specifically, [describe the issue]. Under Apple's Review Guideline 3.1.1, paid apps must function as advertised. I have attached evidence including screenshots and a video recording. I have also attempted to contact the developer, who was unresponsive. I request a full refund. Thank you.

The key is referencing Apple's own guidelines. It shows you did your homework. Most people just click the dropdown menu. Going through the appeals process with a written email signals that you are serious.
